Dental

Premium Plan
Pays 100% of Preventive care (such as cleanings), 80% of basic services (like fillings, simple extractions, root canals, and periodontia), and 50% of major services (like crowns, bridges, and dentures) to an annual maximum of $2,000 per person. Orthodontia expenses are covered for both children and adults to a separate lifetime maximum of $2,000. You may use any dentist in this plan but save using a network dentist.
Network Plan
Same benefits as the Premium plan but you must use a MetLife network dentist.
Economy Plan
Pays 100% of Preventive care (such as cleanings), 60% of basic services (like fillings, simple extractions, root canals, and periodontia), and 50% of major services (like crowns, bridges, and dentures) to an annual maximum of $1,000 per person. Orthodontia expenses are covered for children only to a separate lifetime maximum of $1,000. You may use any dentist in this plan but save using a network dentist.
- Login to your Metlife Account. Print a card, review your claims, etc. Be sure to register if it is your first visit.
- Find a Network Dentist. You may use any dentist in the Economy and Premium plans but using a PDP PLUS Network dentist will give you discounts and cost you less out of pocket. You must use a PDP Plus Network dentist in the Network Plan.
- Dental Claim Form. If you do not have your Metlife ID card or your dentist is “old school,” use this.
Call Metlife at 1-800-942-0854 for more information. You may call Monday–Friday 8 a.m. to 11 p.m. (EST). The service is available in English and Spanish. Automated phone service is available 24/7 for ID cards, network providers, benefits summaries, claim status, etc.
