The vision plan is designed to help employees pay for vision care and materials for themselves and their dependents.
How Does it Work?
This plan pays for eye exams and materials such as frames, lenses, and contacts. use a participating provider and get maximum benefits from the plan. You may use any provider you wish in this plan. However, you will receive special pricing when you utilize an EyeMed network vision provider - including:
- Annual Eye Exam - $20 co-pay (Once every 12 months from the date of service)
- Eyeglass Lenses - $20 co-pay (Once every 12 months from the date of service)
- Frames Allowance - $100 (Once every 24 months from the date of service)
Walmart offers a 15% discount when you use our Walmart discount card. The services are then paid as out of network.
