The CCSD immunization deadline has been extended until October 1, 2021, for ALL students to be up to date on Georgia's state-mandated immunizations (GA Form 3231). Students whose immunizations remain expired on October 1, 2021, will be withdrawn from school.

Note: Students who are 16 and older must have a booster dose of the MVC4 vaccine.

What Should Parents Do?

1. To see if your child is up to date or not, please contact your child's medical provider.
2. Parents do not need to turn any updated records into the schools.
3. Once your child's immunizations are up to date, the school nurse can view the records in GRITS, the state's electronic immunization registry.
